Risk Cascade Modeling of Nutritional Failure and Respiratory Infection in Infants with Labiopalatoschisis and Associated Dysphagia: A Case-Based Framework
Abstract
Infants born with labiopalatoschisis (cleft lip and/or palate) frequently experience a complex interplay of feeding dysfunction, malnutrition, and infectious susceptibility, forming a clinically significant risk cascade that can rapidly progress to severe acute malnutrition (SAM) and respiratory complications. This paper develops a case-based analytical framework for modeling the sequential interaction between dysphagia-induced feeding failure, immune compromise, and respiratory infection, particularly pneumonia. Drawing upon evidence from global nutrition datasets, pediatric infection studies, and cleft-related feeding intervention literature, the study synthesizes a multidimensional risk cascade model that integrates biological, clinical, and system-level determinants. The findings suggest that early feeding dysfunction acts as a primary trigger for nutritional decline, which subsequently amplifies immune vulnerability and increases pneumonia risk. The proposed framework emphasizes integrated pediatric management strategies combining nutritional rehabilitation, airway infection prevention, and structured feeding support. The study contributes to clinical decision-making models for high-risk neonatal populations and highlights critical gaps in early detection and intervention pathways.
